The Orange Health Department has developed a COVID-19 vaccine survey to allocate the COVID-19 vaccine supply according to the phases established by the Connecticut Department of Health. The Orange Health Department will be updating the eligibility criteria based on the guidelines. The location and times of the clinic sites will be posted once the COVID-19 vaccines have been received.
Fill out the form here to register with the Vaccine Administration Management System. You will need to enter your full name, date of birth, eligible occupation (including “retired”) and email address. The information you enter will only be used for purposes of scheduling your vaccination and will be kept private.
Once registered, VAMS will email you to guide you through the appointment process for receiving the COVID-19 vaccine.
At this time, only people eligible under Phase 1a or who are age 75 and over may register. Fill out the form only if you are an Orange resident and eligible to receive the vaccine.
Each individual who completes the survey must have their own unique email to be entered in and vaccinated through VAMS. No duplicate emails can be used. Complete the survey only one time with one email per person.
